Are you planning on participating in a MSHSAA sponsored sport at LHWHS during the 2024-2025 school year? If so, the Activities Office must have a current physical and parent permission on file.

The MSHSAA pre-participation physical exam form is now valid for a two year period from the date of the physical exam (unless otherwise stated by physician), all other pre-participation documents must still be completed/updated annually for every student athlete. Students will not be permitted to participate without all of these documents on file in the Activities Office.

• The first day of fall sports practice is Aug 12, 2024.

• We encourage students to email the needed paperwork to the Activities Office as soon as possible. The forms can be emailed to jstrohbeck@ladueschools.net.

• Athletes will NOT be permitted to official practice without a current physical and parent permission on file with the Activities Office.

• Athletes will receive a participation slip if they have all of the required paperwork. The participation slip must be given to the coach on the first day of practice (Aug. 12). It is their “ticket” to get into practice. More information regarding how to pick-up participation slips will be posted in July. Please visit www.ladueactivities.com for current information.

• As a reminder, all athletes must pay the $75 activity fee prior to their first MSHSAA sponsored event. The fee can be paid after July 1, 2024, by check or online through www.myschoolbucks.com.
