Reimbursement/Payment Request Form

LHWHS-PA volunteers with approved Parent Association expenses may submit a request for payment or reimbursement. Please include all invoices and/or receipts in a single file, preferably in PDF format. As a tax-exempt organization, LHWHS-PA does not reimburse sales tax. Volunteers may request a sales tax exemption letter from the Treasurer prior to making purchases.
